Logan Health is seeking a PRN Pharmacist to support the Specialty Pharmacy in Kalispell, MT. This on-site role provides vacation and schedule coverage as needed and involves direct patient care and medication management within collaborative practice agreements. You will contribute to safe, effective pharmacotherapy in a team-based setting. What You'll Do

Evaluate and verify orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration, and therapeutic redundancy.

Provide patient-specific pharmacotherapy recommendations and drug information to appropriate stakeholders as applicable to assigned area(s).

Perform daily review of pharmacy reports including pharmacokinetic monitoring as applicable to assigned area(s).

Document clinical interventions appropriately in electronic medical record.

Implement all drug use policy and formulary decisions of the Pharmacy and Therapeutics committee.

Review and update Collaborative Practice Agreements per department protocol for assigned area(s).

Support and precept pharmacy students on practice rotations as applicable to assigned area(s).

Assess and evaluate the medical and drug literature. Compare and contrasts therapeutic regimens and uses for new or existing drug treatments.

Provide direct and indirect patient care for patients through Collaborative Practice Agreements as applicable to assigned area(s).

Develop and participate in operational and clinical projects as determined by departmental needs and ongoing projects as applicable to assigned area(s).

Perform direct patient care rounding, order verification, discharge counseling, and education of other healthcare professionals as applicable to assigned area(s).

Supervise the pharmacy technicians as applicable to assigned area(s).

Serve as a preceptor for pharmacy students on rotations as applicable to assigned area(s).

Maintain pharmacy knowledge through continuing education and apply new principles to current practice as needed.

Coordinate medication ordering, preparation, dispensing, and administration systems for the oncology patient populations as applicable to assigned area(s).

-
PharmD from an ACPE-accredited College of Pharmacy, or equivalent clinical experience required.

Current Montana State Board of Pharmacy, Registered Pharmacist License required.

Accurate data entry skills, analytical and advanced math skills required.

RQI Provider (BLS) within 7 days of hire/transfer or before independent patient care, whichever comes first, required.

RQI required and as applicable to assigned area(s): First Responder for assignment: Clinic, Retail, Behavioral Health, IS and Administration Pharmacists. ACLS required for all other assigned area(s).
