About the Role
Cape Fox Concentric Methods is seeking a Pharmacist (Pain Management) to provide comprehensive pharmaceutical services to service members at Naval Hospital Camp Pendleton, California. This role delivers clinical pharmacy care within a multidisciplinary team focused on optimizing pain management and patient education.
What You'll Do

Perform a full range of pharmacist procedures and deliver comprehensive pharmaceutical services within the facility's personnel, supply, and equipment capabilities.

Counsel patients regarding appropriate use of medications and review patient profiles at dispensing to monitor therapy and detect discrepancies.

Intervene with providers for any medication therapy discrepancies; compound medications as required; ensure accuracy of all products prepared by staff.

Supervise inventory control of pharmaceuticals, especially scheduled substances, and maintain security per regulations.

Participate in multidisciplinary teams to provide drug therapy recommendations, polypharmacy management, and patient education on therapy changes.

Conduct medication review and reconciliation for chronic pain patients; develop strategies for non-compliance, abuse or diversion histories.

Conduct clinical interviews to design pharmacotherapeutic treatment plans; order and evaluate labs to assess response, adverse reactions, therapeutic drug levels, and reminders.

Document and communicate findings, agreements, and recommendations; educate patients and families on medications and non-drug therapies; provide written drug information and adherence aids.

Extend existing prescriptions when appropriate and arrange follow-up appointments; order consults and referrals; participate in ECHO tele-mentoring training and tele-health activities.

What We're Looking For

Licensed Pharmacist with clinical expertise in pain management and chronic pain care (or equivalent credentials).

Experience delivering patient-focused pharmaceutical care in a multidisciplinary setting; strong counseling and communication skills.

Ability to review and reconcile medications, assess drug interactions and therapeutic duplications, and manage controlled substances securely.

Capability to work with physicians and other providers to optimize pharmacotherapy; experience with polypharmacy management and non-adherence strategies.

Familiarity with Tele-mentoring/tele-health programs (ECHO) and documentation of treatment findings.

Compensation & Benefits

Salary range: $165,000.00 to $170,000.00 per year.
