About the Role Per Visit Physical Therapist for home health visits in the Southborough/Metrowest region. VNA Care is seeking Per Visit PTs to provide compassionate home health care with Home Health and Oasis E experience. The role offers a daily rate and a flexible schedule with visits scheduled Mon-Fri 8a-4:30p and Sat-Sun 8a-6:30p; training includes working with a preceptor and educator. What You'll Do
- Provide patient care visits in patients' homes as part of the VNA Care Southborough/Metrowest team
- Collaborate with a preceptor and educator; participate in training with a recommended 24-hour weekly schedule during training
- Travel within a defined territory to complete Home Health visits; document care and adhere to Oasis E processes What We're Looking For
- MA PT license in good standing
- Home Health and Oasis E experience
- Willingness to travel within a territory
- Availability to work with a preceptor and educator; able to work a flexible schedule Nice to Have

- Daily rate: $480, varies based on number of visits and types of visits
