About the Role
This Per Diem RN role with The Elliot VNA provides in-home nursing care as part of a nurse-led team serving Manchester, NH and surrounding communities. Our VNA care team delivers individualized in-home care across the continuum to support patients recovering from surgery or coping with life-limiting illnesses or disabilities, with a focus on safety, education, and reducing hospitalizations. Schedule: Per Diem - every other weekend commitment, holiday and on call. What You'll Do
- Comprehensive patient assessment
- Medication reconciliation and education
- Disease process education
- Wound and ostomy care
- Home infusion therapy
- Skilled nursing interventions such as pleural drainage, urinary catheter management, etc.
- Additional skilled nursing interventions as needed
