Position: Participant Care Associate (CNA)
Client: PACE at Hudson Headwaters
Location: Glens Falls, NY
Target Areas: Queensbury, South Glens Falls, Hudson Falls, Fort Edward, Lake George, Saratoga Springs, Ballston Spa, Wilton, Moreau, Gansevoort, Corinth, Warrensburg, Granville, Whitehall, Schuylerville, Greenwich, Argyle, Salem
Pay: $22.50/hour
Schedule: Full-Time; Monday–Friday; 8:00 AM - 4:30 PM; No weekends required
About the Role: Provide direct personal care and support to older adults enrolled in a Program of All-Inclusive Care for the Elderly (PACE), helping maintain safety, dignity, independence, and quality of life while working as part of an interdisciplinary healthcare team.
Key Responsibilities

Provide hands-on personal care (bathing, grooming, dressing, toileting)

Assist with feeding and meal support

Assist with transfers and ambulation

Monitor participant well-being and report concerns

Complete participant care documentation
-
Support participant independence and safety
-
Follow individualized care plans and collaborate with interdisciplinary team members

Maintain participant dignity and comfort
Required Qualifications:

Active New York State CNA Certification
-
High School Diploma or GED
-
Ability to provide hands-on personal care and work with elderly and frail populations

Experience or comfort assisting with bathing, grooming, dressing, toileting, feeding, transfers, ambulation

Ability to work Monday–Friday, 8:00 AM - 4:30 PM

Ability to lift up to 50 lbs occasionally

Reliable transportation

Ability to complete medical clearance and background checks

Strong communication and documentation skills

Ability to work within an interdisciplinary healthcare team
