About the Role As a Home Health Occupational Therapist at CenterWell Home Health in Waco, TX, you will assess and treat patients in their homes to improve strength, mobility and independence. You will collaborate with physicians and the interdisciplinary team to develop and revise plans of care, and you will help patients regain function to perform daily activities with confidence. What You'll Do
- Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
- Administer and interpret functional tests in home or other settings to assist the physician in evaluating function.
- Confer with the physician and health care team to develop/revise the Plan of Care.
- Provide therapeutic treatment and instruction per physician orders to improve/restore strength, coordination, ROM and function, or teach compensation strategies.
- Review and expand retraining programs to align with patient goals within the rehabilitation program.
- Collaborate with vocational team members to develop vocationally oriented plans.
- Report patient status and progress to physicians, rehab staff, Clinical Manager, patient and family.
- Participate in care coordination and discharge planning and support other health care personnel.
- Design community reintegration activities and coordinate with the interdisciplinary team.
- Recommend/design adaptive equipment to improve home/work environments and participation in rehab.
- Design/train staff and family to carry out retraining programs including dressing, feeding, grooming and hygiene.
- Supervise and evaluate OT Assistant and Home Health Aide performance.
- Document care observations, interventions and evaluations; ensure interim orders are documented and implemented.
- Submit evaluations, treatment plans and discharge summaries to the supervisor and care management staff. What We're Looking For
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license, auto insurance and reliable transportation
- Six months of OT experience preferred; Home Health experience preferred Nice to Have
- Home Health experience (preferred) Compensation & Benefits
- Pay Per Visit/Unit Rate: $54.00 - $76.00
