About the Role As a CenterWell Home Health Occupational Therapist, you will assess and treat patients in their homes, helping them regain strength and independence through rehabilitation and functional improvement. You will collaborate with physicians and the care team to develop and execute individualized plans of care and support patients' return to meaningful daily activities. What You'll Do
- Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests of function; collaborate with the physician in evaluating the patient's level of function and in developing Plan of Care.
- Provide therapeutic treatment and instruction per physician orders to improve/restore strength, coordination, range-of-motion and function; teach compensatory strategies.
- Review/expand retraining programs in alignment with patient goals and rehabilitation program guidelines.
- Consult with other vocational team members to develop and implement vocationally oriented plans; participate in care coordination and discharge planning activities.
- Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient and family members; participate in interdisciplinary team communication.
- Design community reintegration activities to assist physical reconditioning and psychological adjustment; coordinate plans with the interdisciplinary team.
- Recommend and/or design adaptive equipment to improve home/work environments and patient participation; design/train staff and family on retraining program skills (dressing, feeding, grooming, hygiene).
- Supervise, instruct and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing services.
- Accurately document care observations, interventions and evaluations; ensure interim orders are documented and implemented; submit evaluation, treatment plans and discharge summaries to supervisor. What We're Looking For
- Current and unrestricted OT licensure.
- Current CPR certification.
- Valid driver's license, auto insurance and reliable transportation. Nice to Have
- Minimum six months of occupational therapy experience.
- Home Health experience. Compensation & Benefits
- Salary: $85,400 - $117,500 per year; pay per visit ranges $54.00 - $76.00 per unit/visit.
