Job title: Occupational Therapist - Home Health PRN
About the Role Join CenterWell Home Health as an Occupational Therapist to help patients regain strength, mobility and independence in their own homes. You will collaborate with the Home Health team to deliver rehabilitation and functional improvement, enabling patients to perform daily activities with confidence.
What You'll Do
- Assess and screen patients' daily living and work-related skills; develop therapeutic retraining programs with measurable objectives.
- Administer and interpret functional tests to help physicians evaluate function in home settings.
- Confer with the patient’s physician and the rehabilitation team to develop and revise the Plan of Care.
- Provide therapeutic treatment and instruction according to physician orders to improve/restore strength, coordination, ROM, and function, or teach compensatory strategies.
- Review and expand retraining programs to align with patient goals and program guidelines.
- Consult with vocational team members to craft vocationally oriented plans.
- Report patient status and progress to physician, rehab staff, Clinical Manager, patient and family.
- Participate in care coordination and discharge planning; act as a resource to other health personnel.
- Design community reintegration activities and coordinate with the interdisciplinary team.
- Recommend/adapt adaptive equipment to improve home or work environments.
- Design and train staff and family to carry out retraining, including dressing, feeding, grooming, and hygiene skills; participate in team/family education.
- Supervise, instruct and evaluate Occupational Therapy Assistant and Home Health Aide performance.
- Thoroughly document care observations, interventions, and evaluations on the day services are rendered; document verbal orders and submit plans and discharge summaries.

What We're Looking For
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license, auto insurance and reliable transportation
Nice to Have
- Minimum six months of occupational therapy experience
- Home Health experience
Compensation & Benefits
- Pay range: $115,300 - $158,600 per year (base pay for full-time)
- Per-visit pay: $79.00 - $109.00 per visit
