About the Role As a Home Health Occupational Therapist at CenterWell Home Health, you will deliver personalized home-based OT services to help patients regain strength, mobility and independence, collaborating with physicians and an interdisciplinary team. This role may serve Loveland and surrounding areas, with emphasis on rehabilitation and functional improvement in patients' homes.

What You'll Do

• Assess and screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function for patients in their homes or other settings to support physician evaluation of function.
• Confer with the patient's physician and health care team; participate in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to improve/restore strength, coordination, range of motion and function or teach compensation strategies per physician orders.
• Review/expand retraining programs in line with patient goals and rehab program guidelines.
• Consult with vocational team members to develop and implement vocationally oriented plans.
• Report patient status and progress to physician, rehabilitation staff, patient and family; participate in care coordination and discharge planning.
• Design community reintegration activities to aid physical reconditioning and psychological adjustment; coordinate with interdisciplinary team.
• Recommend and/or design adaptive equipment for home or work environments.
• Design/train staff and family to carry out retraining programs (dressing, feeding, grooming, hygiene); participate in team and family meetings.
• Supervise, instruct and evaluate Occupational Therapy Assistant and Home Health Aide performance in delivering OT services.
• Document care observations, interventions and evaluations accurately on the day services are rendered; ensure interim orders are documented and implemented; submit evaluation, treatment plans and discharge summaries to supervisor and care management staff.

What We're Looking For

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• (Preferred) Minimum six months of occupational therapy experience.
• (Preferred) Home Health experience.

Nice to Have

• Experience working in home health or community-based rehab settings.
• Ability to collaborate effectively with interdisciplinary teams and families.

Compensation & Benefits

• Pay range: $93,900 - $129,300 per year.
• This is a full-time position, scheduled for 40 hours per week.
