About the Role Become part of CenterWell Home Health and help patients regain strength, mobility and independence in the comfort of their homes. As an Occupational Therapist, you’ll deliver personalized rehabilitation to improve function and quality of life for individuals in need.
What You'll Do
- Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests of function in patients' homes to assist the physician in evaluating function.
- Confer with the patient's physician and health care team and participate in development/revision of the Plan of Care.
- Provide therapeutic treatment and instruction per physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
- Review/expand retraining programs in alignment with behavioral goals and rehabilitation program guidelines.
- Consult with vocational team members to develop vocationally oriented plans consistent with patient needs and capabilities.
- Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient and family members.
- Participate in care coordination and discharge planning activities and act as a resource to other health care personnel.
- Design community reintegration activities to assist the client in physical reconditioning and psychological adjustment, coordinating with the interdisciplinary team.
- Recommend/design special adaptive equipment for clients to improve residence or working environments or participation in the program.
- Design/train staff and family members to carry out the retraining program (including dressing, feeding, grooming and hygiene skills) and participate in team and family educational meetings.
- Coordinate/oversee/supervise/instruct and evaluate Occupational Therapy Assistant and Home Health Aide performance in delivering services.
- Accurately document care observations, interventions and evaluations day by day; ensure interim orders from the physician are documented and implemented; submit evaluation, treatment plans and discharge summaries.
What We're Looking For
- Degree from an accredited Occupational Therapy Program.
- A minimum of six months of occupational therapy experience preferred; Home Health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver’s license, auto insurance and reliable transportation.

- Home Health experience a plus.
Compensation & Benefits
- Salary range: $99,100 - $136,300 per year (USD), with potential variation by location and experience.
