Value Based Care Model - delivering primary care to geriatric panel, Monday - Friday, no weekends or evenings.

Manage chronic conditions of our own patient panel seeing 6-8 patients per day on average. Start day at 8AM with a quick stand-up huddle, ending day around 4:30 p.m. taking no charting in to the evenings using Athena and AI Scribe.

Responsibilities

• The APPs we seek are those who have directly related experience in geriatrics and chronic condition management. The APP who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located

• Federal DEA License/eligibility

• Geriatric training/experience preferred

• Active CPR/BLS Certification

• Board Certification

• Full-time annualized salary paid bi-weekly

• Salary grows year-over-year with annual performance review/merit increase
