Nurse Practitioner – Hospice – Part Time | Bergen County, NJ

Visiting Nurse Association Health Group is seeking a part-time Nurse Practitioner to join our Hospice team serving Bergen County. This position is responsible for ensuring the delivery of high-quality care in the treatment of symptoms and promoting the highest quality of life for patients and their families.

Responsibilities

• Complete required face-to-face encounters and comprehensive physical examinations
• Assess patients for recertification of hospice services
• Evaluate hospice appropriateness using Local Coverage Determination guidelines
• Complete documentation within required timeframes
• Collaborate with the hospice care team regarding patient and family needs
• Serve as a clinical resource to nurses managing complex patient and family needs
• Communicate with patients, families, healthcare agencies, groups, and organizations
• Follow infection control and safety policies

Qualifications

• Master’s degree in Nursing required from an NLN-accredited program
• Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority
• 3+ years of APN experience required
• Excellent communication, organizational, and interpersonal skills
• Advanced computer skills
• Valid driver’s license and automobile insurance

Working Conditions/Physical Demand:

• Working indoors and outdoors with clinical equipment and use of phone and computer. Job characteristics: Client and Customer contact.

• In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $61.00 to USD $72.00 per hour.
