About the Role Nurse Practitioner (Full Time) at Tembo Health will provide remote care to older adults, focusing on dementia care management. You will conduct patient assessments, review medications, and coordinate ongoing care to improve outcomes for patients.
What You'll Do You will conduct assessments of patients and document findings, review medications and reconcile changes, coordinate and manage care plans across care teams, and utilize Athena Health for clinical workflows and documentation.
What We're Looking For We are seeking a family or adult-gerontology NP with at least 1 year of experience. Licensed in AZ, OR, NV or eligible for national licensure. Not excluded from state or federal programs. Passion for dementia care. Comfortable with technology.
Nice to Have Experience with Athena Health or similar EHRs.
