Responsibilities include providing effective and equitable value-based primary care to adults on medicare, conducting office visits for routine and acute issues, administering annual wellness visits and health risk assessments, care coordination with other providers, population health leadership, assisting with phone triage and outreach, educating patients, participating in promotional activities, conducting home visits as needed, and other duties as assigned. The role reports to the Center Medical Director and works closely with operational leadership.
Required Qualifications include Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program, national certification in at least one specialty (Family NP, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP), active non-probationary NP license, active DEA license, and US work authorization.
Preferred Qualifications include experience in primary care (internal medicine, geriatrics, or family medicine), understanding of HCC documentation and ICD-10 coding and HRAs, teamwork, interest in an innovative population health model, and bilingual proficiency.
Anticipated Weekly Hours: 40
