About the Role We are seeking a licensed, experienced Nurse Care Navigator to support our team in Hartford County, Connecticut. This is a part-time, per diem role with flexible hours to be set by the Nurse Care Navigator. The role focuses on assessing client needs, planning care, and advocating for older adults to improve quality of life. What You'll Do
- Assess needs of older adults and plan care; develop thoughtful strategies; advocate for clients; and collaborate with families and the care team.
- Provide care planning advice and coordinate with internal teams to mobilize community resources.
- Uphold mission to improve quality of life and maintain confidentiality of resident, company, and staff information. What We're Looking For
- 10+ years' experience working with older adults.
- Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field.
- LPN or RN required.
- Strong assessment, care planning, advocacy, communication, and computer skills.
- Knowledge of community resources and ability to mobilize them into a coordinated plan of care.
- Valid driver’s license and reliable transportation; willingness to travel as needed.
- Ability to maintain confidentiality; knowledge of health and social factors influencing wellbeing.
- Experience with memory impairment is a plus. Nice to Have
- Experience with individuals with memory impairment is a plus. Compensation & Benefits
- Hourly rate: $60 per hour; per diem; part-time; hours fluctuate and can largely be set by the Nurse Care Navigator.
- The hourly rate is commensurate with experience.
