✅ What You'll Do • Manage patients enrolled in CCM programs • Conduct patient outreach and follow-up calls • Develop and update personalized care plans • Coordinate care with providers and caregivers • Document all patient interactions in eClinicalWorks (eCW) • Educate and support patients in achieving their health goals
🎯 What We're Looking For ✔ CCM experience required ✔ Experience with eClinicalWorks (eCW) ✔ Strong communication and organizational skills ✔ Detail-oriented and patient-focused ✔ Comfortable with phone-based patient engagement
