About the Role
The MOUD Jail Transition Coordinator supports the Clark County Jail Services mission by coordinating direct case management for incarcerated individuals receiving MOUD or on the MOUD waitlist, with the goal of connecting them to community-based MOUD, substance use and mental health treatment, housing, peer support, health care, and basic needs prior to release. This grant-funded role emphasizes individualized release planning and collaboration with jail staff and community partners; the position may begin in 2026 and requires self-direction and organization in a secured environment.
What You'll Do
- Coordinate direct case management services to incarcerated people receiving MOUD or on the MOUD waitlist, linking them to community-based MOUD and related services prior to release.
- Connect clients to housing, substance use and mental health treatment, peer support, health care, and other basic needs.
- Triage clients into case management based on vulnerability (homelessness, age, gender, mental health, substance use, medical needs) and develop collaborative, individualized release plans.
- Coordinate access to community resources and work closely with corrections officers, jail medical and mental health staff, civilians, co-located community partners, and partnering organizations.
- Maintain a high level of self-direction, organization, and work efficiency; participate in potential non-traditional work hours as program needs evolve.
- Document activities and ensure proper reporting and compliance with program requirements.
What We're Looking For

Experience
- Master’s degree in social work, counseling, mental health, behavioral sciences, or related field from an accredited institution plus at least one (1) year of experience working with persons with legal system involvement OR Bachelor’s degree in related field plus at least two (2) years of such experience OR 5 years of equivalent work experience; proof of credentials required; degree conferred by an institution with National Recognition of Accrediting Agencies; transcripts required at hire.
- Preferred Qualifications (optional): Experience with people with substance use disorders (including Opioid Use Disorder) and MOUD; experience with complex co-occurring disorders in adults; experience with case management; experience in correctional facilities; experience working with people during incarceration; knowledge of case management techniques, correctional and reentry practices, trauma-informed care, and related regulations.
- Abilities: Engage effectively and non-judgmentally with incarcerated people; coordinate with internal and external partners; develop collaborative release plans; represent client needs to allied professionals and community organizations; manage services across multiple agencies; exercise professional judgment; employ evidence-based strategies; maintain required documentation; operate within a secured environment and established chain of command.
