About the Role Join Uniting's Community Living Supports team and empower adults living with mental illness to achieve their recovery goals through person-centred, community-based support. This role provides mental health support in community settings across Western Sydney.
What You'll Do
- Provide person-centred mental health support to adults living with mental illness in community-based settings.
- Assist with daily living activities, social integration, and skill development to support recovery goals.
- Collaborate with care teams, family/carers, and service providers to coordinate supports.
- Maintain clear, accurate documentation and progress notes; monitor safety and wellbeing.
- Deliver supports that promote independence, dignity, and inclusion.

- Experience in or a passion for mental health support in community settings.
- Strong communication, empathy, and collaborative skills.
- Ability to work within a multidisciplinary team and adapt to changing needs.
