About the Role Mental Health Clinical Therapist II provides psychosocial assessments, treatment planning and counseling to patients in a Mental Health and/or Substance Abuse setting, coordinating care with families, physicians and community resources to support recovery and safe transitions of care. What You'll Do
- Conduct psychosocial assessments by interviewing patients and significant others to obtain history and identify treatment focus.
- Contact referral sources and/or prior/current care providers to inform enrollment, obtain collateral data and coordinate care.
- Develop, implement and monitor patient’s individualized treatment plan.
- Provide individual, couples and family counseling using various professional modalities.
- Participate in clinical team meetings and contribute to case discussions on psychosocial and disease management needs.
- Inform attending physicians of treatment progress and significant changes in emotional/mental status.
- Petition and transition patients to inpatient care when psychiatric hospitalization is needed.
- Maintain accurate medical records, including psychosocial assessments, family sessions and daily progress notes, with descriptions of interventions and patient response for discharge planning.
- Demonstrate knowledge of community resources to assure post-treatment support and transitions to higher levels of care when needed.
- Represent LOH by speaking at community events and participate in committees to improve clinical care.
- Comply with LOH safety, infection control and security programs. What We're Looking For
- Master’s Degree in a recognized mental health field.
- Knowledge of developmental psychology, disease pathology and clinical needs of the population served.
- Ability to maintain confidentiality and clearly summarize clinical information in written records.
- Ability to demonstrate self-control in volatile situations and maintain therapeutic boundaries with clients during treatment and after termination.
- Current State of Illinois licensure with minimum four letters (e.g., LCPC, LCSW, LMFT) or Psych D.
- Current CPR Certification issued by the American Heart Association.
- Willingness to participate in committees and adhere to ethical standards. Nice to Have
- Minimum of two years of experience in a behavioral health setting. Compensation & Benefits
- Hourly pay range: $32.60 - $48.90 (USD).
