- Enter MAC-related personal information: first name, last name, birth date, email, phone number, and ZIP Code
- Provide and verify a DoD ID number (10 digits)
- Optionally enter referrer information, including their DoD ID and contact details for status updates
- Indicate ROTC interests and connect with appropriate contacts
- Consent to receive email communications from the U.S. Army, and consent to receiving texts from recruiters
- Acknowledge and review the Waiver & Release of Liability and Privacy Policy
- Review and comply with Privacy Act Notice and related legal disclosures What We're Looking For
- Willingness to provide accurate personal and contact information
- Ability to supply a valid DoD ID (10-digit)
- Willingness to consent to email and SMS communications from recruiters
- Understanding and acceptance of privacy and liability notices
