Responsibilities

• Provide nursing care to residents in accordance with physician orders and established care plans.
• Administer medications and treatments as prescribed.
• Monitor residents’ health status and promptly report changes to the appropriate healthcare professionals.
• Accurately document nursing assessments, treatments, medications, and resident care.
• Assist with coordinating resident care and communicating with physicians, families, and interdisciplinary team members.
• Follow infection control procedures, safety standards, and facility policies.
• Support resident dignity, comfort, and overall well-being.
• Assist with admissions, transfers, and discharges as needed.

Qualifications

• Current and active California LVN license.

• Previous skilled nursing or post-acute care experience preferred.

• Knowledge of nursing practices, medication administration, and infection prevention.

• Strong communication, organizational, and clinical skills.

• Ability to work effectively as part of a multidisciplinary team.
