About the Role The Clinical Practice Nurse at Concord Hospital delivers high quality patient care including assessment, treatment, planning, and care evaluation under the direction of practice providers. The role emphasizes patient-centered nursing across the life span and collaboration with the care team.
What You'll Do
- Establish priorities in organizing daily activities and maintain efficient patient flow.
- Triage patient phone calls following established guidelines to identify health problems and provide quality care.
- Administer prescribed medications.
- Provide education to patients in response to identified needs.
- Document health information, interventions, and outcomes into the patient electronic medical record.
- Delegate patient care activities using the five rights and provide feedback to the delegatee and manager.
- Apply the nursing process to make sound clinical judgments and decisions.
- Complete practice-specific clinical skill competencies annually.
What We're Looking For
- Education: Associate Degree or Diploma in Nursing from an accredited School of Nursing.
- Licensure: Current New Hampshire Board of Nursing license.
- Certification: Current American Heart Association Basic Life Support for Healthcare Providers.
- Experience: Ability to interact effectively with a multi-disciplinary staff, patients, and others.
