Job title: LPN | Med Surg | 36 Hours | Nights
About the Role The Licensed Practical Nurse (LPN) provides direct patient care in the Med/Surg unit under supervision of an RN, ARNP, physician, or physician assistant. The role includes patient assessment, treatment, care planning, and ongoing evaluation to promote optimum health outcomes. This position is based in Concord Hospital's Laconia/Franklin, NH locations.
What You'll Do
- Triage patient phone calls following established guidelines and data collection formats.
- Document health information, interventions, and outcomes in the EMR.
- Provide health education to patients to promote optimum health.
- Administer prescribed medications according to the five rights of medication administration.
- Perform physical and psychosocial assessments and plan care accordingly.
- Complete annual clinical skill competencies and perform other nursing tasks within scope.
- Perform other nursing services commensurate with LPN experience, education and demonstrated competencies.
What We're Looking For
- Completion of a Licensed Practical Nursing program from an accredited School of Nursing; NH LPN licensure.
- Current New Hampshire LPN license.
- Certification required: Current American Heart Association Basic Life Support for Healthcare Providers or Red Cross CPR/AED for Professional Rescuers and Health Care Providers.
- Demonstrated ability to effectively and positively interact with a multi-disciplinary staff, patients and others.
- Willingness to perform nursing duties commensurate with experience and demonstrated competencies.
