About the Role This LPN position at Logan Health's Conrad Long Term Care provides skilled nursing care to patients within the LPN scope of practice and collaborates with the care team to support the organization's mission and values. The role requires professionalism, compassion, and adherence to patient privacy and safety in the assigned unit(s). What You'll Do
- Provide skilled nursing care to patients using the nursing process in the assigned area(s).
- Administer medications, treatments, and provider-specific care in accordance with nursing policies/procedures and LPN scope of practice.
- Perform standardized focused assessments or reassessments at regular intervals as directed by the RN and provider orders.
- Communicate all relevant changes in patient condition to the RN, provider, and other stakeholders in a timely manner.
- Document clinical findings and required regulatory information in the medical chart.
- Maintain an updated, individualized plan of care including goals, interventions, medications, and visit frequency as applicable.
- Adhere to infection control practices and protect patient privacy and confidentiality.
- Operate and maintain equipment to departmental standards and support cost-containment and safety.
- Demonstrate professionalism, civility, teamwork, and excellent customer service in all interactions. What We're Looking For
- Current Montana LPN license (or LPN Compact) required.
- RQI Provider (BLS) within 7 days of hire or before independent patient care, whichever comes first.
- Minimum 1 year of experience in Long Term Care or Critical Care setting.
- Valid driver’s license and driving record with no major offenses in the prior 36 months; state minimum liability coverage for personal vehicles used for Logan Health business.
- Strong organizational, verbal and written communication, interpersonal, and confidentiality skills.
- Proficiency with Microsoft Office and ability to learn other software as needed.
- Ability to work well in a team and independently and to think critically and prioritize tasks. Nice to Have
