Looking for a nursing role where you’re a leader, not just another body on the schedule? Three Meadows Post Acute in Perrysburg is hiring a 2nd shift LPN to join a supportive team that puts residents first. On second shift, you set the tone for the evening: guiding your CNAs, caring for residents, and keeping the unit running smoothly.

What You’ll Do

• Provide hands-on nursing care to residents, including medication administration, treatments, wound care, and monitoring changes in condition
• Lead your unit’s evening shift by supervising and supporting nursing assistants and making daily rounds
• Be the link between residents, families, and physicians, including physician orders, family updates, and care coordination
• Keep care plans on track, working with the care plan team and contributing to quality assurance
• Help new residents settle in, from admission and orientation to discharge planning
• Promote a safe, infection-free environment and help staff follow safety and infection control practices

What You’ll Bring

• A graduate of an approved LPN program
• Current, unencumbered Ohio LPN license in good standing with the State Board of Nursing
• Current CPR certification
• Leadership skills and a team-first attitude
• Patience, tact, and compassion for working with older adults and their families
• Ability to lift 50 lbs and move through the shift comfortably
• PointClickCare (PCC) experience is a plus, but not required
