About the Role Responsible for direct referral source service delivery, providing information on programs and services to grow existing business and develop new business and coordinating referrals to ensure optimal transitions from facility to home. What You'll Do
- Collaborate with clinical and hospice teams to promote smooth transitions from facility to home and maintain open communication with VNA Care and referral sources.
- Manage direct referral source servicing by maintaining key accounts, building new business, identifying needs, problem solving and meeting or exceeding expectations of referral sources.
- Educate key targets on the benefits and quality of care for patients using VNA Care and elicit feedback regarding programs and services.
- Develop and implement a sales and marketing plan for home health, palliative care and hospice services to enhance account relationships and increase referrals.
- Develop and maintain relationships with physicians, referral sources, managed care and assigned accounts; assist with specialty programs.
- Represent VNA Care in meetings, presentations, educational programs and conferences, and coordinate with referral sources as needed.
- Develops and maintains a work plan and prepares reports and maintains records per agency/department expectations.
- Monitor external and internal environment to assess opportunities, increase market share and gather competitive information.
- Coordinate efforts with Regional Manager and clinical colleagues to exceed service delivery and provide service excellence to external and internal customers.
- Respond promptly to customer complaints and participate in resolution to ensure complete service recovery.
- Assist other liaisons with account coverage as needed.
- Meet or exceed referral and admission targets.
- Uphold compliance objectives and policies and procedures of the Foundation and Subsidiaries.
- Perform other related duties of a similar nature and complexity as directed. What We're Looking For
- Education: Associate’s Degree in a clinical field required; Bachelor of Science in a clinical field preferred.
- Licensure: Licensed to practice in Massachusetts.
- Experience: Minimum of 3 years clinical experience with 1 year in home health care.
- Skills: Strong communication, relationship-building, and problem-solving abilities; experience in sales/marketing for healthcare and account management. Nice to Have
- Bachelor’s degree (preferred) and experience with physician engagement and managed care. Compensation & Benefits
- Salary: $100,000 - $115,000 per year (compensation based on experience).
- Location: On-site at a Boston hospital; on-site support and collaborative team environment.
