About the Role VNA Care is seeking licensed practical nurses (LPNs) to join our North Shore Home Care team on weekday schedules (Monday–Friday), with no weekend shifts. This role focuses on delivering high-quality home-based nursing care to patients and supporting their health goals in the comfort of their homes. What You'll Do
- Provide skilled nursing care to patients in their homes according to physician orders and care plans.
- Assess patient conditions, monitor progress, document findings, and communicate changes to the care team.
- Administer medications, manage wounds, wound vacs, catheters, ostomies, and perform phlebotomy as appropriate.
- Support patients with chronic conditions such as CHF, COPD, and diabetes through education, symptom management, and care coordination.
- Maintain accurate charting, complete timely documentation, and participate in orientation and ongoing training. What We're Looking For
- Completion of an LPN program and licensure to practice in Massachusetts.
- Ability to work Monday through Friday with no weekend shifts; willingness to receive fully paid orientation.
- Experience with wounds, wound vacs, catheters, ostomies, phlebotomy, CHF, COPD, and diabetes is helpful. Nice to Have

- Rate of pay: $32.69 - $47.60 per hour, plus mileage reimbursement.
