About the Role The Lakes Home Care is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care to pediatric and/or adult patients in their own homes. This per diem home health role supports the care plan and coordinates with the health care team to deliver compassionate, high-quality care in patients' familiar environments.
What You'll Do
- Provide skilled nursing services within the scope of practice and according to the plan of care, in coordination with the health care team.
- Assess, plan, implement, and evaluate patient care; observe responses to treatment and report changes to the supervising nurse or physician.
- Instruct patients’ families on care needs and maintaining a healthy home environment.
- Observe, document, and communicate the patient’s status and any nursing diagnoses.
- Adapt to various home settings and maintain professionalism in all interactions.
What We're Looking For
- Current and unrestricted LPN/LVN licensure in the state of practice.
- Current TB test or Chest X-Ray and current BLS/CPR certification (CPR within 90 days if not current).
- Valid Driver's License, access to a dependable vehicle or reliable transportation.
- Home care experience and ability to work independently; strong communication and organizational skills.
- Flexibility to accommodate varying schedules and patient needs.
Nice to Have
- CPR certification if not already current and ability to complete within 90 days of hire.
- Prior home health experience; proven reliability and adaptability.
Compensation & Benefits
- Per diem pay: $21.11 - $46.68 per hour; annual total cash target pay $43,900 - $97,100 for full-time productivity.
- Two compensation streams: per-visit point pay and hourly pay for non-visit activities (e.g., orientation).
