As the LPN MDS Coordinator, you will play a vital role in ensuring accurate resident assessments, care planning, and reimbursement processes while supporting the delivery of exceptional care. Your work will directly contribute to the health, well-being, and quality of life of those we serve.
What You'll Do

Coordinate and complete the Minimum Data Set (MDS) assessment process in accordance with federal, state, and organizational regulations.

Collaborate with interdisciplinary team members to gather assessment data and ensure comprehensive resident evaluations.

Assist in the development and updating of individualized care plans based on resident needs and assessment findings.

Monitor assessment schedules and ensure timely completion and submission of all required documentation.

Review clinical documentation for accuracy, completeness, and regulatory compliance.

Support quality measures and performance improvement initiatives.

Serve as a clinical resource for MDS-related regulations, documentation requirements, and reimbursement processes.

Work closely with nursing leadership to promote excellence in resident care and regulatory compliance.

What You'll Bring

Required Qualifications
-
Current Licensed Practical Nurse (LPN) license in the State of Maryland.

Minimum of one (1) year of nursing experience in long-term care, skilled nursing, rehabilitation, or a related healthcare setting.

Strong knowledge of nursing documentation, resident assessment practices, and care planning.

Excellent organizational, analytical, and communication skills.

Ability to manage multiple priorities while meeting deadlines.

Ability to read and speak English.

Preferred Qualifications

Previous MDS Coordinator or MDS experience.

Experience with PointClickCare or other electronic health records systems.

Knowledge of quality measures and regulatory compliance requirements.

Compensation & Benefits
Competitive Salary:
- 70,000 - 90,000 annually (Final compensation will be based on experience, credentials, and demonstrated expertise.)
