Job title: Licensed Practical Nurse (LPN) - Conrad Long Term Care
About the Role This position provides practical nursing care to patients within the scope of practice and as applicable to the assigned area(s). It supports Logan Health's mission and values as it delivers quality, compassionate care.
What You'll Do
- Provides skilled nursing care to patients utilizing the nursing process as appropriate for LPNs in the assigned area(s).
- Administers medications, treatments, and provider-specific care in compliance with nursing policies and LPN scope.
- Communicates timely and effectively any changes in patient condition to RN, provider, and other stakeholders.
- Documents all clinical findings and regulatory requirements in the medical chart.
- Maintains and updates an individualized plan of care including goals, interventions, medications, and visit frequency.
- Maintains infection control procedures and adheres to safety policies.
- Operates and maintains equipment to departmental standards; supports cost containment.
- Demonstrates professionalism, civility, teamwork, and patient-centered service in all interactions.
- Participates in shift activities and ensures regular attendance as scheduled; shift: Night Shift - 12 hours; schedule may adjust to meet organizational needs.
What We're Looking For
- Current Montana Licensed Practical Nurse (LPN) license or LPN Compact license.
- RQI Provider (BLS) within 7 days of hire/transfer or before independent patient care.
- 1 year of experience in Long Term Care or Critical Care setting.
- Driver's license with driving record free of major offenses in the past 36 months; state minimum liability coverage for personal vehicles used for Logan Health business.
- Strong communication, organizational, critical thinking, and ability to work both independently and as part of a team.
- Proficiency with Microsoft Office Suite and ability to learn other software as needed.
