About the Role The Intake Nurse - RN is responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their families. The role also involves knowing required outreach activities within the community and assessing eligibility and safety of potential members. Collaboration with the Intake/Outreach team and the clinical team ensures appropriate provision of services to new members and a seamless transition to managed long term care.
What You'll Do
- Conducts outreach in the community as needed
- Performs the initial assessment using the UAS (Uniform Assessment System) tool
- Evaluates potential members for eligibility and safety
- Determines the care needs of the potential member and formulates the initial plan of care
