About the Role
We’re seeking a Part-time Physician to provide in-home health evaluations for Signify Health, part of CVS Health, delivering personalized health assessments to members in the comfort of their homes.
What You'll Do

Conduct 45-minute in-home visits including medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by the member's health plan.

Use iPad and clinical workflow; no prescribing, no lab orders, no changes to current treatment unless directed by care plan.

Travel locally and within licensed states; conduct virtual visits as needed; provide Diagnostic & Preventive Services as indicated.

Collaborate with Signify Health clinicians and health plans to connect members to the right care.

What We're Looking For

MD or DO with active, unrestricted license in coverage area(s).

Ability to obtain multi-state licenses (preferred).

Willingness to travel to primary locations including Genesee, Lapeer, Lenawee, Livingston, Macomb, Monroe, Oakland, Saint Clair, Washtenaw, Wayne.

Experience conducting in-home evaluations and working in home environments with diverse conditions.

Compensation & Benefits

Pay: $105.00 per hour (range is the same for this role).
