About the Role
Join Signify Health, part of CVS Health, as a Full-Time Physician delivering in-home health evaluations. The clinician visits members in their private homes to assess health status, review medications and medical history, and coordinate next steps with the health plan to improve care delivery.
What You'll Do

Conduct approximately 45-minute in-home visits, connecting one-on-one with members

Review medication and medical history, perform a physical evaluation, and document findings

Perform additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry)

Use an iPad and a clinical workflow; you are not required to prescribe medication, order labs, or alter the plan without guidance

Travel locally within licensed states and conduct virtual visits as needed

Provide Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening) as indicated

Coordinate with health plans, PCPs, and care teams to connect members with appropriate care

Work within a national network of clinicians to support health outcomes

What We're Looking For

MD or DO with active, unrestricted licenses in the coverage area(s); multi-state licenses preferred

Ability to travel locally; flexible schedule; anticipated weekly hours about 40

Comfortable performing visits in home environments (may include pets and other household conditions)

Proficiency with iPad-based clinical workflows; ability to conduct virtual visits when needed

Direct pay: typical range $125.00 per hour
