About the Role

Signify Health, part of CVS Health, is seeking a Full-time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and bridging gaps in care. You’ll join a national network of clinicians dedicated to improving health outcomes by delivering patient-centered evaluations to people’s doorsteps.

What You'll Do

• Conduct in-home health evaluations (approximately 45 minutes) including medication and medical history review, physical examination, and, if ordered by the person’s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry).

• Use an iPad and an intuitive clinical workflow for each evaluation; you are not required to prescribe medications, order labs, or alter the patient’s current treatment plan.

• Be flexible to travel locally and within licensed states; conduct virtual visits as needed.

• Provide Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).

• Join a national network of purpose-driven clinicians and contribute to connecting patients with the right care.

• What We're Looking For

• MD or DO with active, unrestricted license(s) in the relevant coverage area(s).

• Multi-state licenses (preferred).

• Ability to cover primary locations: Clinton, Eaton, Ingham, Jackson, Shiawassee (MI).

• Anticipated 40 hours per week; full-time role.

• Willingness to travel locally and within licensed states; comfortable performing in-home visits (home environments may include pets, secondhand smoke, and other potential allergens).

• Willingness to conduct virtual visits as needed.

• Nice to Have

• Experience with in-home health evaluations or similar home-based care delivery.

• Experience with spirometry or other diagnostic testing is a plus.

• Comfort with using tablets and digital clinical workflows.

• Compensation & Benefits

• The position offers a typical pay range of $125.00 per hour.
