About the Role

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations, delivering personalized care to patients in their own homes.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the patient’s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). You’ll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicines, order labs, or alter patients’ current treatment regimens.

What You'll Do

• Conduct in-home health evaluations (~45 minutes) including medication/history review, physical examination, and selective diagnostic testing per health plan.

• Use Signify Health’s iPad-based workflow; focus on assessment and care coordination rather than prescribing or changing treatments.

• Travel locally and within licensed states as needed; conduct virtual visits when appropriate.

• Provide Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening) as indicated.

• Build trusted relationships with patients and connect them to the right care across the health plan.

• Work collaboratively with Signify Health clinicians to fulfill the role responsibilities.

• Conduct visits in patients’ homes, which may include pets and various environmental conditions; adhere to safety and quality standards.

• What We're Looking For

• MD or DO with active, unrestricted license(s) in the coverage area(s).

• Multi-state licenses preferred.

• Anticipated weekly hours: 40.

• Willingness to travel locally and within licensed states; capable of virtual visits when needed.

• Excellent communication and patient-centered care approach.
