About the Role
Signify Health, part of CVS Health, is seeking a Full-time Physician to deliver In-Home Health Evaluations in members' homes, helping bridge gaps in care. Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and, if ordered by the health plan, diagnostic tests. Clinicians use an iPad with a streamlined clinical workflow; prescribing, ordering labs, or altering current treatment regimens is not required.
What You'll Do

Conduct in-home visits (~45 minutes) to assess health needs, review medications and medical history, and answer questions.

Perform physical examinations and, when indicated by the health plan, order or facilitate diagnostic tests (e.g., spirometry, diabetic eye exams).

Use Signify Health’s clinical workflow on an iPad and integrate findings with the patient’s care team.

Travel locally to visit members at their homes; provide virtual visits as needed.

Provide Diagnostic & Preventive Services as indicated and help connect patients to the right level of care.

Work in members’ homes (primary workplace); environments may include pets, secondhand smoke, and other conditions.

What We're Looking For

MD or DO with active, unrestricted license(s) in the coverage area(s).

Multi-state licenses (preferred).

Ability to cover Grand Rapids area (Counties listed: Allegan, Barry, Ionia, Kent, Mecosta, Montcalm, Muskegon, Newaygo, Oceana, Ottawa).

40 hours per week; full-time role.

Must be able to travel locally and work in home environments.

Hourly pay: $125.00 per hour (base).
