About the Role

Signify Health, part of CVS Health, is seeking a Full-time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. You’ll play a critical role in building trusted relationships to make people healthier, bringing flexible, comprehensive and personalized health evaluations right to people’s front doors.

What You'll Do

• Conduct 45-minute in-home visits to review medication and medical history, perform physical evaluations, and order optional diagnostic tests if indicated.

• Use an iPad and a streamlined clinical workflow; no requirement to prescribe meds, order labs, or change current treatments.

• Travel locally and within licensed states as needed; conduct virtual visits when required.

• Provide all Signify Health services including Diagnostic & Preventive Services (e.g., spirometry, DEE, etc.).

• Build trusted relationships with patients, care teams, and PCPs to connect people with the right care.

• What We're Looking For

• MD or DO with active, unrestricted license(s) in the applicable coverage area(s); multi-state licenses preferred.

• Ability to cover Emmet, MI (Charlevoix, Cheboygan, Emmet, Presque Isle) and surrounding counties; anticipated weekly hours around 40; full-time.

• Willingness to travel locally; ability to perform visits in patients' homes (home environments may include pets or secondhand smoke).

• Proficiency with iPad and clinical workflow; comfortable conducting virtual visits as needed.

• Strong communication skills and dedication to improving health outcomes.

• Multi-state licenses (preferred).

• Compensation & Benefits

• Salary: $125 per hour.
