You will conduct comprehensive assessments in patients' homes, bridging gaps in care without altering current treatment plans, using an iPad-based workflow. What You'll Do
- Conduct 45-minute in-home visits including medication and medical history review, physical evaluation, and, when ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry).
