About the Role

As a physician with Signify Health, part of CVS Health, you will provide in-home health evaluations to members in Alpena, MI and surrounding licensed areas. You will conduct comprehensive assessments in patients' homes, bridging gaps in care without altering current treatment plans, using an iPad-based workflow. This full-time role offers travel within licensed states and the opportunity to contribute to a national network focused on improving health outcomes.

What You'll Do

• Conduct 45-minute in-home visits including medication and medical history review, physical evaluation, and, when ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry).

• Use an iPad and a clinical workflow to document visits; you are not required to prescribe medications or order labs or change patients' treatment plans.

• Travel locally and within licensed states as needed; conduct virtual visits when appropriate; connect patients to the right care and coordinate with health plans and PCPs.

• Provide Diagnostic & Preventive Services as indicated (e.g., spirometry, DEE).

• Maintain professional relationships and adhere to safety and health standards within diverse home environments.

• What We're Looking For

• MD or DO with active, unrestricted medical license in the designated coverage area; multi-state licenses preferred.

• Ability to cover Alpena and surrounding counties (Alcona, Alpena, Crawford, Iosco, Montmorency, Ogemaw, Oscoda, Otsego, Roscommon) and travel locally as needed.

• 40 hours per week; ability to work full-time schedule.

• Strong communication skills and comfort visiting patients in their homes; capable of using digital tools (iPad) and a clinical workflow.

• Commitment to patient-centered care and working within health-plan-driven care teams.

• Nice to Have

• Multi-state licensure (preferred)

• Experience with in-home health evaluations and remote/virtual care settings
