About the Role
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for health plan members in home settings. Visits are typically ~45 minutes and include a medication/history review and physical evaluation; diagnostic tests may be ordered by the member's health plan. You’ll use an iPad and a streamlined clinical workflow, and you are not required to prescribe medications, order labs, or alter current treatment regimens. The primary workplace is the member’s residence, and clinicians travel locally and within licensed states; virtual visits are possible as needed.
