About the Role
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for health plan members in home settings. Visits are typically ~45 minutes and include a medication/history review and physical evaluation; diagnostic tests may be ordered by the member's health plan. You’ll use an iPad and a streamlined clinical workflow, and you are not required to prescribe medications, order labs, or alter current treatment regimens. The primary workplace is the member’s residence, and clinicians travel locally and within licensed states; virtual visits are possible as needed.
What You'll Do

Conduct in-home health evaluations (~45 minutes) in members' homes using an iPad and the clinical workflow

Review medication and medical history, perform physical evaluation, and, if applicable, coordinate recommended diagnostic tests

Travel locally and within licensed states as needed; conduct virtual visits when required

Provide Signify Health services including Diagnostic & Preventive Services (e.g., spirometry, bone density screening, etc.)

Build trusted relationships to connect members with appropriate care and improve health outcomes

Operate as a Signify Health clinician, adhering to role requirements and safety standards

Note: Not required to prescribe medications or change current treatment regimens

What We're Looking For

Active, unrestricted NP or PA license in your coverage area(s)

Board certification (required)

Anticipated Weekly Hours: 12; part-time availability

Ability to travel locally and within licensed states as needed

Demonstrated communication skills and compassionate patient engagement

Pay: $41.85 - $90.13 per hour
