About the Role

CVS Health's Signify Health is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for plan members, bringing assessments directly to patients' homes in Marquette, MI. The visits focus on medication and medical history review, a physical exam, and optional diagnostic tests ordered by the member's health plan, with no obligation to prescribe or change current treatment during the visit.

What You'll Do

• Conduct ~45-minute in-home visits with one-on-one engagement to review medications and medical history, perform a physical examination, and complete any health-plan–ordered diagnostic tests (e.g., diabetic eye exams, spirometry).

• Use an iPad and Signify Health's clinical workflow; travel locally and within licensed states as needed; may conduct virtual visits as required.

• Bring compassionate care coordination for members and help connect them to appropriate care, while working within the network of Signify Health clinicians.

• Provide all Signify Health services indicated, including Diagnostic & Preventive Services (e.g., spirometry, DEE facilities, etc.).

• Note: You are not required to prescribe medication, order labs, or alter a member's current treatment plan during these visits.

• What We're Looking For

• Active, unrestricted nurse practitioner (NP) or physician assistant (PA) license in the coverage area(s) with board certification (required).

• Multi-state licenses preferred.

• Able to cover the following primary locations: Marquette, MI (Counties: Alger, Baraga, Marquette).

• Anticipated Weekly Hours: 29.5.

• Able to travel locally and within licensed states; virtual visits as needed.

• Compensation & Benefits

• Pay range: $41.85 - $90.13 per hour (USD).
