About the Role

In-Home Health Clinician (Nurse Practitioner or Physician Assistant) Part-time role providing in-home health evaluations for Signify Health in Iron Mountain, MI. Primary workplace is the private residence of health plan members; these environments may include pets, secondhand smoke, and other conditions. You’ll join a national network of purpose-driven clinicians dedicated to improving health outcomes.

What You'll Do

• Conduct ~45-minute in-home visits to connect one-on-one, review medication and medical history, perform a physical evaluation, and address health questions.

• When requested by the member’s health plan, perform additional diagnostic tests (e.g., diabetic eye exams, spirometry) and document findings in the clinical workflow.

• Use an iPad and Signify Health’s clinical workflow; you are not required to prescribe medicines, order lab tests, or alter current treatment regimens.

• Be flexible to travel locally and within licensed states as needed; conduct virtual visits as needed.

• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry).

• Collaborate as a clinician colleague within the Signify Health network to connect members with appropriate care.

• What We're Looking For

• Active, unrestricted license(s) in coverage area(s) (Iron Mountain and surrounding counties: Dickinson, Iron, Menominee).

• Board certification (required).

• Ability to cover the stated primary locations; anticipated Weekly Hours: 29.5.

• Multi-state licenses (preferred).

• Compensation & Benefits

• Pay range: $41.85 - $90.13 per hour (hourly).
