About the Role

In-Home Health Clinician (Nurse Practitioner or Physician Assistant) for Signify Health, part of CVS Health, delivering in-home health evaluations to members in their own homes. This part-time role focuses on connecting with individuals to improve health outcomes by assessing needs and coordinating care.

What You'll Do

• Conduct in-home health evaluations lasting about 45 minutes, including a medication and medical history review and a physical examination, plus additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).

• Use an iPad and a streamlined clinical workflow; you are not required to prescribe medicines, order labs, or alter patients' current treatment plans.

• Travel locally within licensed states as needed to visit members at home; virtual visits may also be conducted as needed.

• Provide Diagnostic & Preventive Services as indicated by the health plan.

• Join a national network of clinicians dedicated to improving health outcomes and connecting members to appropriate care.

• What We're Looking For

• Active, unrestricted license(s) in the coverage area(s) (e.g., Michigan, Gogebic, Houghton, Keweenaw, Ontonagon).

• Board certification (required).

• Multi-state licenses (preferred).

• Able to cover primary locations: Houghton and the specified counties.

• Anticipated weekly hours: 29.5.

• NP or PA credential; ability to travel locally to member homes.

• Compensation & Benefits

• Salary range: $41.85 - $90.13 per hour.
