About the Role

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to deliver In-Home Health Evaluations to health plan members in the comfort of their homes. You will help bridge gaps in care by delivering personalized health assessments and building trusted relationships with patients.

What You'll Do

• Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry).

• Use an iPad and a clinical workflow for each evaluation; not required to prescribe medicine, order labs, or alter current treatment regimens.

• Travel locally and within licensed states as needed; conduct virtual visits when appropriate.

• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening).

• Join a national network of clinicians dedicated to improving health outcomes and connecting members to the right care.

• Primary workplace is the private residence of health plan members; environments may vary and require safe, adaptable care.

• What We're Looking For

• Active, unrestricted Nurse Practitioner or Physician Assistant license in the coverage area (state(s)) and board certification (required).

• Multi-state licenses (preferred).

• Ability to travel locally and conduct in-home visits; comfortable performing home-based evaluations.

• Anticipated weekly hours: approximately 29.5 hours.

• Strong communication and relationship-building skills to engage with patients in their homes.

• Nice to Have

• Experience performing in-home health evaluations and delivering preventive services in home settings.

• Additional licensure or certifications that support in-home assessments and spirometry/diagnostic testing.

• Compensation & Benefits

• Pay range: $41.85 - $90.13 per hour (hourly).
