About the Role

A Part-time Clinician (Nurse Practitioner or Physician Assistant) will provide In-Home Health Evaluations for Signify Health, part of CVS Health, delivering care in members' homes to bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests.

What You'll Do

• Conduct in-home health evaluations lasting about 45 minutes, connecting one-on-one with members

• Review medication and medical history, perform a physical examination, and document findings

• Order diagnostic tests when indicated by the member's health plan (e.g., diabetic eye exams, spirometry)

• Use an iPad and a clinical workflow; not required to prescribe medications, order labs, or alter current treatment

• Travel locally as needed and conduct virtual visits as needed

• Join Signify Health as a clinician colleague, fulfilling role requirements

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services (e.g., spirometry, DEE)

• What We're Looking For

• Active, unrestricted license(s) in the coverage area(s)

• Board certification (required)

• Multi-state licenses (preferred)

• Ability to cover the primary locations: Sault Ste. Marie, MI (Chippawa, Luce, Mackinac counties)

• Anticipated Weekly Hours: 29.5

• Willingness to travel locally to meet health plan members at home

• Nice to Have

• Multi-state licenses (preferred)

• Experience performing in-home or community-based health evaluations

• Compensation & Benefits

• Salary range: $41.85 - $90.13 per hour
