About the Role
A Part-time Clinician (Nurse Practitioner or Physician Assistant) will provide In-Home Health Evaluations for Signify Health, part of CVS Health, delivering care in members' homes to bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests.
What You'll Do
- Conduct in-home health evaluations lasting about 45 minutes, connecting one-on-one with members
- Review medication and medical history, perform a physical examination, and document findings
- Order diagnostic tests when indicated by the member's health plan (e.g., diabetic eye exams, spirometry)
