About the Role
Signify Health, part of CVS Health, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in Alpena, MI. The role involves visiting members at home and delivering flexible, comprehensive, personalized health evaluations.
What You'll Do

Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, and a physical exam.

Perform additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).

Use an iPad and clinical workflow; not required to prescribe medications, order labs, or alter current treatment plans.

Travel locally and within licensed states as needed; conduct virtual visits as required.

Provide Diagnostic & Preventive Services products (e.g., spirometry, bone density screening).

Join a national network of clinicians focused on improving health outcomes and helping people access the right care.

Work in the member's home environment, which may include household pets, secondhand smoke, and other potential allergens.

What We're Looking For

Active, unrestricted license(s) in the applicable states in coverage area(s).

Board certification (required).

Multi-state licenses (preferred).

Anticipated weekly hours around 29.5; part-time availability.

NP or PA credentialed clinician; ability to travel locally.

Excellent communication skills and compassion for patient-centered care.

Compensation & Benefits

Pay range: $41.85 - $90.13 per hour.
