About the Role

We are seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for Signify Health, part of CVS Health. You’ll visit patients in their homes, engage with them, and help bridge gaps in care by delivering personalized health evaluations.

What You'll Do

• Conduct approximately 45-minute in-home visits, including a medication and medical history review and a physical evaluation.

• Perform additional diagnostic tests when ordered by the patient’s health plan (e.g., diabetic eye exams, spirometry).

• Use an iPad and a streamlined clinical workflow; no requirement to prescribe medications, order labs, or alter current treatment plans.

• Travel locally and within licensed states as needed; conduct virtual visits as required.

• Provide all Signify Health services, including Diagnostic & Preventive Services products, as indicated.

• Build trusted relationships with members and coordinate care connections to the right services.

• What We're Looking For

• Active, unrestricted license(s) in the coverage area(s) and board certification (required).

• Multi-state licenses (preferred).

• Full-time availability (~40 hours per week).

• Ability to travel locally; strong communication and patient-education skills.

• NP or PA credentialing as applicable; experience in home-based or community health settings is a plus.

• Nice to Have

• Multi-state licensing (preferred).

• Prior experience with home visits or home health evaluations.

• Compensation & Benefits

• Salary range: $87,035 – $187,460 per year.
