About the Role
We are seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for Signify Health, part of CVS Health. You’ll visit patients in their homes, engage with them, and help bridge gaps in care by delivering personalized health evaluations.
What You'll Do
- Conduct approximately 45-minute in-home visits, including a medication and medical history review and a physical evaluation.
- Perform additional diagnostic tests when ordered by the patient’s health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad and a streamlined clinical workflow; no requirement to prescribe medications, order labs, or alter current treatment plans.
