About the Role

You will join Signify Health, part of CVS Health, as a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for members, bringing care directly to their front doors. The role focuses on connecting people with the right care in a flexible, home-based setting.

What You'll Do

• Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical examination, and any recommended diagnostic tests as ordered by the health plan.

• Use an iPad and Signify Health clinical workflow; you are not required to prescribe medicines, order labs, or alter current treatment regimens.

• Travel locally and within licensed states; conduct virtual visits as needed.

• Provide all Signify Health Diagnostic & Preventive Services.

• Build trusted relationships with members to help connect them with the right care for their unique needs.

• Work as a clinician colleague within a national network, contributing to improved health outcomes.

• Adapt to home environments, including potential exposure to pets, secondhand smoke, and other factors.

• What We're Looking For

• Active, unrestricted license(s) in the relevant NP or PA scope of practice and board certification (required).

• Multi-state licenses (preferred).

• Willingness and ability to travel locally and perform visits in members’ homes; comfortable working in diverse environments.

• Strong communication skills, patient-centered approach, and commitment to safety and quality.

• Anticipated 40 hours per week.

• Compensation & Benefits

• Salary range: $87,035.00 – $187,460.00 per year.
