About the Role
You will join Signify Health, part of CVS Health, as a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for members, bringing care directly to their front doors. The role focuses on connecting people with the right care in a flexible, home-based setting.
What You'll Do
- Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical examination, and any recommended diagnostic tests as ordered by the health plan.
- Use an iPad and Signify Health clinical workflow; you are not required to prescribe medicines, order labs, or alter current treatment regimens.
- Travel locally and within licensed states; conduct virtual visits as needed.
