About this role

About the Role The Housing Navigator will support Medi-Cal beneficiaries experiencing homelessness through Elica Health Centers' CalAIM Community Supports program by linking clients to housing resources and developing individualized housing plans that promote stability and reduce reliance on high-cost services. This role works under the Community Supports Supervisor to help clients achieve safe, permanent housing and connect with needed services. What You'll Do

  • Link eligible Medi-Cal clients experiencing homelessness to housing resources and develop a housing plan focused on breaking the cycle of homelessness and sustaining housing stability.
  • Provide participant advocacy, engage with landlords and other agencies, coordinate with Case and Care Managers to ensure linkage to benefits, and connect with Mental Health or Substance Use Services as needed.
  • Offer individualized support to help each client address barriers and sustain permanent housing; provide Housing Transition Navigation, Housing Deposits, Housing Tenancy, and Sustaining services as part of CS programs.
  • Ensure program compliance and collaborate with the CS team (and ECM/other support programs); document encounters and maintain HMIS data entry using VI-SPDAT assessments where applicable.
  • Work with health plans and community partners to coordinate housing-related services, deposits, and related supports. What We're Looking For
  • High school diploma and 1+ year housing navigation or case management experience.
  • CPR and First Aid (or willingness to train); valid California driver’s license and auto insurance.
  • Experience with EPIC and HMIS; ability to document detailed encounter notes.
  • Bilingual Spanish/Farsi/Russian preferred.
  • Ability to collaborate with community housing agencies and landlords; willingness to meet clients out of the office or clinic environment as needed.
  • Knowledge of Coordinated Entry System, tenant screening, housing assessment, and HMIS data entry; experience with VI-SPDAT.
  • Ability to develop Needs Assessments and Individualized Housing Stabilization Plans, set goals, and track tasks.
  • Ability to work with health plans on housing deposits and related services (CS, ECM, medical, dental, behavioral health, transportation, interpretation, etc.). Nice to Have
  • Bilingual in Russian, Farsi, or Spanish (preferred). Compensation & Benefits
  • Salary: $25-$27 per hour.
