- Assess patients’ individual needs using medical information and personal interviews.
- Develop plans to help patients adjust to medical diagnoses and transition from healthcare facilities to everyday life.
- Provide support based on patients’ health conditions, personal circumstances, and emotional needs.
- Research local support groups and resources suitable for individual patients.
- Evaluate the effectiveness of services and resources provided to patients.
- Follow up with patients throughout diagnosis, treatment, and recovery to assess their mental and emotional well-being.
- Stay updated on local resources and relevant medical research to provide appropriate recommendations.
