About the Job

The Team Leader is a non-exempt, hybrid role (on-site and remote) reporting to the Department Team Manager. You will bridge the gap between administrative leadership and hands-on care, balancing daily clinical operations, call triage, and staff scheduling with occasional field visits. You will collaborate closely with management to ensure the high-quality, cost-effective delivery of community health services.

What You’ll Do

• Coordinate & Schedule Care: Match patients with field staff based on acuity and competency, optimizing scheduling for clinical efficiency, staff engagement, and continuity of care.
• Triage & Communicate: Field incoming calls from clients, families, and vendors while processing phone orders from physician offices.
• Support Field & Admin Operations: Perform field clinical visits as needed to deliver hands-on care, participate in weekend/holiday rotations, and assist management with quality improvement initiatives.
• Lead Hospice Initiatives (If applicable): Manage patient pharmacy needs, oversee DME ordering, and lead Interdisciplinary Team (IDT) meetings when the manager is absent.

Who You Are:

• Licensed Professional: Registered Nurse with a current New Hampshire Board of Nursing license and active CPR certification.
• Experienced Clinician: Holder of an Associate’s degree (BSN preferred) with 3 to 5 years of clinical experience, including at least 1 year in Home Care or Hospice.
• Mobile & Reliable: Possessor of a valid state driver's license, reliable auto insurance, and a clean criminal background record.
• Strong Communicator: Skilled at multitasking triage needs, navigating scheduling software, and collaborating with cross-functional healthcare teams.
